All children under age 16 must apply for a passport in person with two parents or guardians.
You cannot renew your child's passport.
Passports for children under age 16 are only valid for 5 years.

Required items for a Child Passport
1) Photocopy, Front and Back, of Parent's Valid Driver's License or State ID
- If your ID was issued in the last 6 months or is expired, provide at least two additional IDs.
2) Evidence of Citizenship - (Must provide 1 of the following)
- Original or certified copy of your U.S. birth certificate and expired or about-to-expire passport book if one was issued previously
- Certificate of Naturalization
3) Passport Fees - (Check or Money orders only made payable to the U.S. Department of State.)
- $182.05 payable to the U.S. Department of State for a new 10-year passport book
- $197.05 payable to the U.S Department of State for passport book and passport card
- The fee includes 1-2 day shipping from the Philadelphia Passport Agency
4) Passport Photos - (CVS, Walgreens, Staples, Post office, or Clerk's office)
- In color
- No glasses
- Taken within the last 6 months
- In front of plain white or off-white background
- Neutral facial expression with both eyes open
- Religious Head wear you will need a Signed Statement
- Do not wear a hat or head covering that obscures the crown or hairline
5) Travel Itinerary - (Print 2 copies)
- Travel plans must be confirmed and include your name, departure date, and flight details.
- Print your confirmation flight itinerary, emailed to you by your travel provider.
- Hotel Reservations (Can only be used for driving across the border, Canada, or Mexico.)
- Business Travelers (Provide a Letter from your employer on company letterhead stating the reason for emergency travel.)
6) Letters of Authorization
- You must Print 2 forms and complete by hand with a pen.
- Hold the extra copy on the side for Prompt Passports
- This will allow us to submit the passport application on your behalf.

- The non-applying parent must complete this form, get it notarized, and provide a photocopy of their ID front and back.
9) Schedule Your Passport Acceptance Agent Appointment
To process your passport application, an official passport acceptance agent must first verify your identity and witness your signature. Once they do, they will seal your application and supporting documents in an official envelope. You are then responsible for shipping or dropping this sealed envelope to Prompt Passport Services.
It’s crucial that you get your identity verified within 7 days of submitting your application, or it will expire.
